A few things about this survey and how it works

In order to work, this survey uses something called cookies, so the option to allow cookies needs to be switched on in your web browser (n.b. a web browser is the software you use to view pages on the internet, e.g. Internet Explorer, Firefox etc...).

When you accessed this page, it performed a quick test to see if your browser does accept cookies. Here is the result:

Cookies not accepted - error!
I think there may be problem here. As far as I can make out, your browser is not accepting cookies from this website. That means you will need to change some settings in order to take the survey.

If you would like some more information about how to change the settings on your computer that relate to cookies, or about the use of cookies on this site, please click here.

Please note that for best results, your screen resolution should be set to at least 1024 by 768 pixels (Windows users can adjust this setting from the Display tool in the Windows Control Panel). Lower resolutions may cause the pages to display incorrectly.

Although every attempt has been made to make this survey compatible with as many different types of computer and web browser as possible, we couldn't cover every eventuality. Most common configurations are supported – if the survey renders strangely for you, please accept our apologies.

This survey contains a total of 34 separate pages - this may seem a little daunting, but they are quite small and will not take long to complete.

Most of the questions require an answer. For these questions, if you do not provide an answer, you will not be able to proceed to the next page.

If you find that you are short on time and need to stop, you will be able to return at a later date to complete the survey. All you need to do is to close down the browser window you are using to view the survey. You can then return to this site anytime up to 7 days later, and carry on from where you left off. Important: this will only work if you are using the same computer and the same browser. Don't forget to bookmark the site in case you need to come back later.

Please do not use the 'Back' button on your browser to attempt to view the questions and answers on previous pages. Only your first submitted answers will be stored, and trying to view previous pages may in fact cause you to miss other pages. If you feel that you gave an answer you are unhappy with on a previous page, you can use the 'Cancel' button to exit the survey completely. Please note that this will delete all the answers you have given so far, and you will have to start from the beginning again.
